Thanks for making this form so much easier to submit. Last time I tried (a year or so ago) my computer refused to put it in send mode in the email. Then again, maybe it was me!! Anyway, this was so much easier.

I have enjoyed being the Temporary Quilt Registration Goddess, as both my Mother and Father have Alzheimers.
This is my first quilt. I pledged a while back under a different email address. I will continue to make more.

Not sure if you registered for the $1,000 Promise or the Quilt-A-Month-Club, but if you’ve changed your email address since then, please tell the person administering the program and give them your OLD email address and your NEW email address.

I’m trying to plan ahead. Will I still need to sew labels on the back — one with the name of the quilt, the maker, and the date, and a second with the registration number? Thanks for your time. ~Laura
One label should do it. Place it on the back of your quilt in the lower right hand corner and just leave room to ink in the quilt number nice and big.

I am going to be the “Quilt Registration Goddess” for my guild later this Spring.

I agree–this form is great and so much easier to use…leaves more time for quilting
It’s probably right there in front of me but I don’t see how to refresh the form to submit a second registration. I was able to get it back by returning to the “Make a Donation Quilt” page and clicking on the link posted there. Is there a simpler way?
It should refresh after you click SUBMIT. If not, go to another page and come back to it.
